1. How to become New Patient
The first step to become a new patient at the Surgical Movement Disorders Clinic is to contact our office at (415)-353-2311 or have your physician fax a referral to (415)-353-9060. Please include our new patient referral request to expedite the review of your records.

2. Requesting an Appointment
Be sure to have your current neurologist or primary care physician fax or send your medical records, including MRI films, to us before the date of your visit.

3. Health Insurance
Be aware that many insurance plans will require an official insurance referral from your primary care physician before you can schedule a visit with a specialist at UCSF. Contact your insurance to learn whether you need to obtain an authorization prior to your visit.

4. What to bring with you to appointments


Sending Radiology Images

The UCSF Department of Radiology Imaging Library offers a secure and convenient way for you to electronically send your images to our clinic via web. If you are interested in sending us your images via ReferralBox, please read this document for more information and then contact the clinic for an email notification to be sent to you.
